A CIA agent within the Central Intelligence Agency refers to someone who is recruited by a CIA officer to spy for US intelligence. In the outside world, CIA agent can also be used to describe someone who is an officer of the Agency, an American who works for the CIA.
Within the intelligence community, CIA agent is the reference to a Soviet or Afghani or French person who has been recruited to spy on his or her own country and report to the CIA case officer.
